What is the Vial of Life?
The Vial of Life is a community safety initiative designed to help paramedics and emergency responders quickly find essential medical information about you during an emergency.
Your medical details are placed in a clearly marked packet or container, stored in a visible location in your home–often on the refrigerator–so responders can locate it within seconds.
The program started out with pharmacies giving out pill vials that your medical information could be stored in. Las Vegas Fire and Rescue's program uses the Vial of Life Magnetic Folder so that it can be more easily noticed by all.
What to include in the Vial of Life
- Completed medical information form
- Copy of insurance cards
- Copy of state ID
- Copies of any advanced directives
How to use the Vial of Life
- Fill out the Vial of Life form that is included. More forms can be found here.
- Place the form and any other pertinent information in the folder provided.
- Place the folder on the refridgerator using the attached magnetic strips.
- Place the provided Vial of Life sticker on your front door or window so responders know where to look.
Why it matters
In an emergency, you might not be able to speak for yourself.
The Vial of Life allows emergency responders to immediately access:
- Allergies
- Medications
- Medical conditions
- Emergency contacts
- Hospital preferences
- Advanced directives.

Where to get your Vial of Life Folder
Las Vegas Fire and Rescue distributes the Vial of Life folders through the Community Paramedic Program.
If you have a large group of individuals that would benefit from these folders, please call our administrative office to be connected with Community Paramedics and schedule a distribution at 702-383-2888.
Vial of Life folders also are available to be picked up at any city of Las Vegas fire station or our administrative office front desk, 500 N. Casino Center Blvd.
